Clinical Analysis of Blunt Abdominal Trauma in Childhood



Abstract
This report was clinical analysis of 40 patients who were operated upon for blunt abdom-inal trauma in the Department of General Surgery, Wonju Christian Hospital, during the period of 5 years from January 1979, to December, 1983.
The results obtained were as follows.
1) The case with laparotomy for blunt abdominal trauma was 3.24% to total trauma of children under 15 years of age in same period.
2) The childhood trauma characterized as the ratio of male to female 5:1, most commonage distribution at age 5~9, frequent in July, August, September.
3) The cause of trauma was mainly car accident recording 62.5%.
4) The hemoglobin levels showed 10~15 mg% in 21 cases(52.5%) and white blood cell counts, over 20.000/mm3 in 15 cases(37.5%).
5) Diagnostic paracentesis has 93.1% in accuracy for determining the presence of abdom-inal injury mainly hemoperitoneum.
6) As to the organs injured and the frequency of involvement, the spleen and liver were the most common injured organs in 13 cases each, followed by kidney in 10 cases, retrope-ritoneum in 8 cases.
7) 15 patients had associated extraabdominal injuries such as chest injury, the most com-mon type, bone fracture, etc.,
8) Complication rate was 25% with 5 cases of pulmonary complication, the most common type.
9) Death occured in 8 patients, a mortality rate of 20%.
The mortality was more susceptable to younger age group, female patient, and to the case of associated injuries. The most common cause of death was multiple organ failure in 4 cases.
